Why do mass defects exist?

Systemic recalls happen when a defective component — an airbag inflator, a fuel pump, an electronic control module — was installed in multiple models from multiple brands over years, before the defect was detected in the field. The result: tens of thousands of vehicles affected under different badges, but with the same root defect.
